It is the philosophy at Evolved Behavioral Health Services Outpatient Program to assist persons with developing and enhancing current skills that facilitate adjustment and integration within their living environment and community. Members are empowered and encouraged to make decisions about their care to increase their quality of life. 

Evolved LLC Behavioral Health Services

Evolved is a fee-for-service Outpatient Clinic. Outpatient programs are provided in a clinic setting. A wide range of behavioral health services are scheduled and organized within a comprehensive therapeutic environment which include screening and assessment, individual counseling as needed, group counseling sessions, educational programming, member advocacy, as well as participation in community resources. A licensed clinician is available on site to assist with individual planning and care as needed. Day programs are coordinated and facilitated by professional care providers such as experienced behavioral health services technicians and other consultation Professional Care providers. 

Our evolved outpatient programs assist persons with developing and enhancing current skills such as:

  • Substance Abuse / Relapse Prevention
  • Socialization Support / Interpersonal Communication
  • Mental Health Building Healthy Coping Skills

Goals and Objectives

The overall goal of Evolved outpatient program is to increase the quality of life of our members through the provision of behavioral health services in Tucson. Specific areas of focus include building healthy coping skills, socialization support, anger management, and interpersonal communication development.

Evolved outpatient program seeks to achieve objectives such as improving member mood regulation and socialization skills, reduce the need for higher levels of care, and supporting members as they work towards independence.

Behavioral Health Services

Evolved provides help that ensure all members are evaluated and are provided behavioral health services based on their individual needs. The specific areas that demonstrate a continuum of care are as follows:

  • Screening and Assessment: A screening and intake process is completed within 14 days of the initial visit. The member's unique needs and preferences are identified and assessed into an individual behavioral health services treatment plan.
  • Service Planning: An individualized service plan is created with each member within 14 days of entry to the program. Members are full participants in their planning process, and goals and objectives are based upon their strengths and preferences for treatment. Goals are presented in client-friendly language and are available to them upon request.
  • Therapy Services: Members have available to them a masters level licensed clinician who will meet with members to build specific individual therapy goals. Therapy sessions are provided at least once per month.
  • Psychosocial Education: Psychosocial Education is available to each member in both group and individual formats based upon their specific needs. Psychosocial interventions aim to build interpersonal communication, anger management, role performance, treatment compliance, and communication skills.
  • Progress Reviews: Evolved will work with established referral treatment teams to update on member progress and to revise individualized plans as necessary. Staff are available by telephone or are able to travel (if enough notice is given) to a referral agency. All to help you understand the services being received.
