Mental Health Services Tucson Can Trust

Evolved provides a continuum of mental health services that ensures that all members are evaluated and provided services based upon their individual needs. The specific areas that demonstrate a continuum of care are as follows:

  • Screening and Assessment: A screening and intake process is completed within 14 days of the initial visit. The member's unique needs and preferences are identified and assessed into an individual treatment plan.
  • Service Planning: An individualized service plan is created with each member within 14 days of entry to the program. Members are full participants in their planning process, and goals and objectives are based upon their strengths and preferences for treatment. Goals are presented in client-friendly language and are available to them upon request.
  • Therapy Services: Members have available to them a masters level licensed clinician who will meet with members to build specific individual therapy goals. Therapy sessions are provided at least once per month.
  • Psychosocial Education: Psychosocial Education is available to each member in both group and individual formats based upon their specific needs. Psychosocial interventions aim to build interpersonal communication, anger management, role performance, treatment compliance, and communication skills.
  • Progress Reviews: Evolved will work with established referral treatment teams to update on member progress and to revise individualized plans as necessary. Staff are available by telephone or are able to travel (if enough notice is given) to a referral agency.

Payment for Services

100% of the payment for mental health services in Tucson is due at the time of the provided services unless the services are contracted. Cash, checks, and money orders are accepted from individuals who self-pay for all or part of their services. We also offer a sliding fee scale for self-pay members. A receipt will be issued for all types of payments, and all payments are recorded in a ledger and forwarded to the accounting department for data entry. Payment will be refunded to the client if a third party provides reimbursement to the agency. Requests for refunds are forwarded to the accounts receivable for administrative review. If approved, refunds are paid no later than 30 days from the date of the request by a company check.

Continued Stay Criteria

  • Emotional / behavioral conditions continue to be treated.
  • Progress on treatment goals noted, but acceptance / understanding not yet sufficient to maintain self-directed recovery program.
  • Member continues to develop positive recovery environment.
  • Harm-reduction strategies are being employed by the treatment team.

Discharge Planning and Criteria

Discharge planning happens throughout all phases of the programming. Members meet discharge criterion when their individual goals have been met, are unsuccessful due to inappropriate behavior, lack of attendance, or non-compliance with treatment plan. Individuals may also request discharge or move out of location or if an individual request to be transferred to an alternative agency for treatment. Additionally, if a member loses their funding source for the requested services, they may be discharged from their mental health services.

Referral Criteria

Evolved will refer any member on the basis that we do not provide specific services for, and will not maintain a waiting list.

Mechanisms to Address Special Population

Evolved addresses the special needs of members using an individual treatment planning process that highlights a person’s strengths, abilities, needs, preferences, and desired outcomes. These individual mental health services treatment plans will be monitored and modified as appropriate with input from the member and their treatment team / guardian.

Non-English-Speaking Members

Evolved will make arrangements for translation services as necessary when appropriate to support the member's mental health services treatment participation.

Accessibility: the Evolved facility is in compliance with the requirements specified in section 504 of the American with Disabilities Act. Evolved has been modified to support members with mobility impairment, sensory impairment, or other physical disabilities. There is an assessable path from parking lot to all facilities including access to restrooms, doorways, water coolers, and meeting rooms.

Discontinuation of Mental Health Services

Under certain circumstances, it may become necessary for Evolved to discontinue, deny treatment, or to transfer a member from Evolved to other mental health services.

It is the purpose of this policy to ensure the safety and well-being of members and staff and to maintain the therapeutic mission of the program.

Any decision that affects the member's care will be communicated to the member and the referring treatment team. All forms and notices, including a right to appeal the agency decision, will be provided to the affected member.

Conditions under which treatment / participation in a specific mental health service may be discontinued or denied:

  • Threats or acts of violence towards staff or other members of the program
  • Criminal behavior on agency property
  • Use of alcohol or other drugs on agency property
  • Inability or unwillingness to maintain attainable treatment goals.
  • Ability but refusal to pay for services
  • Inability to meet admission criteria
  • A specific program is at capacity
  • Refusal to sign necessary releases and other program paperwork necessary for continued treatment.
